Crisis Services

Crisis services include: Crisis Intervention Hotline, Mobile Crisis, Urgent Care appointments at the Outpatient Clinic in Wisconsin Rapids, Adult Protective Service Investigations, processing of guardianships/protective placements.

Crisis Intervention

Telephone service available 24/7. Crisis Interventionists are available to provide a listening ear, problem solve stressful situations, assess need for and arrange Urgent Care appointments in the Outpatient Clinic, approval for mental health detentions, and initiating Mobile Crisis response. Mobile Crisis

Certified under Chapter 34, Subsection III. Available from 8 a.m. to 12 midnight week days and 11 a.m. to 7 p.m. weekends and holidays to assist law enforcement and others assess a mental health crisis for suicide risk and appropriateness of enacting a detention or arranging a diversion. Urgent Care Appointments

Appointment with a Mental Health therapist available daily for individuals experiencing a mental health crisis. Need for appointment screened and arranged by contacting the Outpatient Clinic receptionist or Crisis Intervention .

Adult Protective Services

An Adult-at-Risk is someone who is believed to be, or at risk of being, abused, financially exploited, neglected, or self-neglecting. To request an investigation of a potential Adult-at-Risk -an adult, age 18-59, who has a developmental disability, traumatic brain injury, serious and persistent mental illness, or a like disability resulting from the consumption of alcohol or substances, contact Unified Services Central Intake.

Guardianship/Protective Placements

Assistance for individuals with a developmental disability, traumatic brain injury, serious and persistent mental illness, or substance abuse issue who are legally incompetent to obtain a court appointed guardian of person, estate, or both. Referrals for this service are made by contacting Unified Services Central Intake.
