The following services will provided for patients upon admission included in their
all-inclusive daily rate:
- Patient will be seen within 48 hours of admission by the Medical Director of Pathways
to do initial health and physical.
- Medical Director for up-dated health and physicals will see patient every 30 days,
for the first two months of their stay and every 60 days thereafter; or on an as
needed basis.
- Pathways Treatment Director will evaluate patient within first week of admission
and on an as needed basis thereafter.
- Patient will have a behavioral plan/assessment written by Pathways Treatment Director.
- Patient will have weekly care-conference, Pathways Treatment Director will determine
with patient if it is beneficial to attend care conference.
- Patient will be seen by Pathway’s Psychiatrist, on an as needed basis, to determine
if medication needs to be reviewed i.e. modified, discontinued etc.
- Patient will be provided with nursing care by Pathways Nurses with the following
but not limited to i.e. dressings, syringes, stockings, treatments, etc.
- Patient will be provided with the medications that are prescribed to them by pharmacy.
- Patient will be scheduled to participate in therapies i.e. Speech, Occupational
and Physical two to three hours a day with Pathways Therapist. Patient will be evaluated
by therapy staff to determine what therapist are seen as a highest priority need
for patient.
- Patient will have the opportunity to participate in daily scheduled as well as non-scheduled
activities, on and off the unit, which will be carried out by Pathways COTA, Certified
Occupational Therapist Assistant, as well as Pathways CNAS, Certified Nursing Assistants.
- After patient is cleared to participate in vocational services by Medical Director,
as part as the health and physical exam, patient will than be evaluated by Pathways
Vocational Coordinator when warranted to determine if patient would benefit from
being involved in work skills at facility or community based.
- The patient will be provided with room and board while on the Pathways unit.
- Facilities dietary department as prescribed by the physician will provide patient’s
meals and nourishment.
- Patient will have use of wheelchair, walker, cane, etc. deemed necessary by staff.
- Patient will have his/her laundry services completed by facility’s laundry department.
- Patient will have stocked toiletries available to him/her i.e. shampoo, toothpaste,
toothbrushes, deodorant, etc.
- Patient will be transported from facility to appointments, outside of facility,
for appointments scheduled by our facility.
- Patient will have access to a phone on the unit as well as a cordless phone to take
to their room for privacy.
Services and supplies not covered by the daily rate:
- Private duty nursing
- Ambulance
- Dental work, dentures, dental exams, etc.
- Any laboratory work or x-rays processed outside of Pathways
- Beautician or barber services
- Professional services other than the staff at Pathways
- Hearing aids, eyeglasses, prostheses, razors, etc.
- Cosmetics and toiletries not stocked
- Purchase of personal clothing and dry cleaning
- Replacement of lost or damaged clothing
- Replacement or repair of valuable and personal belongings
- Meals and purchases for activities outside of Pathways
- Telephone in room
- Television in room
