Pathways Unit - Admission Process

Criteria for Admission
- Criteria for admissions will be based on an individualized process.
- Age range -- Individuals served will have a requirement age of 18. Expectations
may be made if there are no appropriate alternatives available, and it is established
that we can appropriately meet all the needs of the individual. Procedures outlined
in HFS 132 for admitting minors will be followed in these cases, and an Educational
Plan will be developed.
- Level of functioning parameters:
- Individuals will no longer be in a coma.
- Individuals will be alert.
- Individuals will be able to benefit from therapies.
- Individuals will be sufficiently medically stable to function in a Nursing Home
setting.
- Individuals will be able to breath on their own.
- Individuals will not be utilizing esophagus-feeding tubes but may use gastric feeding
tubes.
- Must have a brain injury, acquired or traumatic, to be admitted to the Brain Injury
Unit.
- Patient must be either deemed competent or if deemed incompetent temporary guardianship
must be petitioned, before being placed on the Pathways Unit.
- Ability to sustain at least a minimum of two hours of therapy a day; as well as
benefit from therapy.
- Look at the Brain Injury Unit’s population demographics and ensure the patient is
a good fit with the current cliental being served at the time of acceptance.
