Effective July 1, 2011, ForwardHealth will begin using a new non-emergency medical transportation manager, LogistiCare. You will no longer be able to schedule rides or receive reimbursement for rides through county offices or Economic Support.

If you are a member of Family Care, you will need to contact your case manager at CCCW. Members residing in a nursing home will need to contact the home they are residing in. For all other members:

Call LogistiCare at 1-866-907-1493
to arrange all of your medical rides scheduled on or after July 1st.
Phone calls will be accepted beginning June 17th.

This change in transportation services does NOT affect your eligibility, enrollment or benefits for Wisconsin Medicaid or BadgerCare Plus.
